Hardback register with black canvas covers and maroon colored spine and corners with gold borders. The spine says "Records of Ministerial Acts" in gold lettering, above two small stickers/labels with dates and instructions. There is wearing along all the edges, especially the spine. The spine is completely loose from the book along the front left edge, but attached along the back. The leather and book binding are visible underneath the loose spine. There are four small loose "membership pledges" inside the front cover. All of the records were kept in cursive ink. Due to the "chapters" of different ministerial acts, the page numbers printed in the register do not correspond to the actual page in the Internet Archive.
