5" x 7" booklet when folded, 10" x 7" when unfolded, constructed from heavy card stock from the Henderson Lithograph Company in Cincinnati, OH. The front and back covers are colored a bluish-gray background. The front cover has a large clock face set at 11:00, with the head of an elk in the upper left corner, and a standing angel along the right side of the clock. Written in the clock face are the words "Cervus alces" which is the Latin genus and species name for the American Elk. Underneath the title "Elks Memorial Service," is a stamp for the "Astoria Lodge No. 180, B.P.O.E." This service was held in the Independent Order of Odd Fellows (I.O.O.F.) Building. The back cover lists those who passed away, the officers, the trustees, and the committee members. Inside is the Order of Exercises, conducted by Mrs. Charles.H. Abercrombie (Lucile Ford) along with the pianist Miss Anna Campbell.
